Here is a general feedback letter sample that you can use for whatever purpose, provided that you fill in the right details.

Thank you for {doing something positive. Always start with a positive notion when giving feedback. Let the person know how much you value them and their skills, and be as precise as possible. Like, I love the fact that you never miss a deadline.}

I would also like to draw your attention to the {issue at hand. Then explain how that issue affects you, the team, and the company. Example: I would also like to draw your attention to the fact that many published content pieces weren’t edited or checked before submission. That hurts our relationship with the clients who have pointed this out to us.} 

{Envelop the negative in a form of advice and offer an immediate solution. Example: I understand your drive to submit the tasks as quickly as possible, but maybe you could use an online tool or a spell-checker as part of a routine article submission checklist.} Let me know your thoughts on the matter, and let me know if there’s anything that I, as a manager or {Company}, can do to help you turn this around.
