Follow Up Letter to Request Information

You have sent an email to the person requesting some information, and you want to check if they have seen and acknowledged the request. Since you haven’t received a response back, now you want to send a follow-up letter to request information. Here is an editable, vague template that you can use as a basis for your letter.  

Dear {Recipient’s Name},

I hope you are doing well. I have sent an email on {date} regarding {the issue you have}, and I was wondering if you had a chance to look at it. Given that I still haven’t received a response back, I just wanted to make sure that you had seen the email and that I could follow up with you about it. If you could let me know at your earliest convenience, that would be much appreciated.

If you haven’t had a chance to take a look at it yet, or if you need more details, please let me know.
